T-Shirt and Cap Bulk Order Form
Order your T-Shirts and Caps in bulk quantities for discount prices by filling in this form. Before you click Submit, please review the notes below (additional charges apply). If you would like to mail in your order instead of using this online form, please print out a paper order form.
Please Review Before Proceeding:
- Orders should be submitted by three (3) weeks prior to your event. If orders are received any later, then you may not receive your order until the day before your event.
- Bulk orders must total 12 or more items for the discounted pricing.
- Shipping charges will be added to this order (8% of subtotal or $10 minimum).
- There is an additional $10 handling fee for orders of less than 3 dozen shirts.
- You will receive an email with a copy of the invoice and expected shipping date details shortly after placing your order.
- All orders will ship on a Tuesday. Please allow 1-3 business days for UPS shipping.
- Sales Tax does not apply for sales outside Tennessee. For Tennessee-based churches or other tax-exempt organizations: Please send a copy of your organization’s Certificate of Sales Tax Exemption in order to be exempted from Sales Tax. Email to bobbie@operationinasmuch.org.
